You could carry a:

P7, or a small 1911, or a DA/SA hammered gun, and cover the hammer w your thumb, or a revolver.

I have a couple AIWB holsters. I move the muzzle end, along with the rest of the holster, to the 3:00 position and use it as a standard IWB. Problem solved.
