Can anyone tell me what app I can place on a phone in order to only allow access to specific websites I list?
Most software I am looking at lets you block specific sites or categories of sites, but there are way to many for that to ever work 100%.

You are wanting to have a whitelist of allowed sites... Only thing I know of would be an enterprise level device management suite, any app you download is going to be removable by the user, they can simply turn it off or uninstall it. So you would have to lock down the phone/tablet. I am sure there are apps that claim to do it, but my guess is that any that do not cost at least 3 or 4 figures (server cost plus client license) are going to do a horrible job at it. And really if you are only looking at doing it for one device, there may not be a solution.
